Advance Care Planning

This webinar is Part II of a two-part review of issues related to health care consent and advance care planning, presented by Judith Wahl of the Advocacy Centre for the Elderly (ACE). It has information on what advance care planning is, who can do it, and how to do it. It reviews how advance care plans are used in the consent process, who the advance care plan "speaks to," and what to do if a substitute decision maker (SDM) is not following a patient's advance care plan. This webinar also looks at what SDMs can do if they don't understand the advance care plan, or need guidance to interpret it, or believe that, if capable, the patient would not want the SDM to follow the advance care plan.
